The Support at Home program

How Support at Home works

Support at Home is the Australian Government program that funds services helping older people keep living at home. Here is the path most people follow.

  1. Assessment

    You contact My Aged Care and have an assessment of your needs.

  2. Funding

    The government approves a level of funding based on that assessment.

  3. Care plan

    You choose a provider and together you agree what support looks like.

  4. Support

    Your services begin, delivered by workers you have met.

  5. Reviews

    Your plan is reviewed and adjusted as your needs change.

It is an estimate to help you plan a conversation, not a quote, and not a guarantee of entitlement. Your actual contribution depends on your circumstances and current government rules.

Thinking of moving?

Changing providers is more straightforward than most people expect

You are allowed to change Support at Home providers, and your funding moves with you. You do not need a reason, and you do not have to have a difficult conversation on your own.

  • Your funding follows you

    The approved package belongs to you, not to your current provider.

  • We handle the handover

    With your consent we coordinate the transfer so services continue.

  • Start with a conversation

    Ask questions first. Nothing changes until you decide it should.

For health professionals

Referring a patient or client

We accept referrals from GPs, hospital discharge teams, allied health professionals, social workers, community organisations and family members. Tell us what the person needs and we will follow up directly with them.

    Our nurses work alongside the person’s GP and specialists.

Common questions

Questions people ask us first

Support at Home is the Australian Government programme that funds services helping older people keep living in their own homes, things like personal care, nursing, domestic help, transport and social support. Rather than paying for everything privately, approved services are funded through the programme, usually with a contribution from you. Our Support at Home page explains it in plain English.

Eligibility is determined by the government, not by providers. It generally starts with contacting My Aged Care and having an assessment of your needs. We can explain the process and what to expect, but we cannot approve funding or promise an outcome. That decision sits with the assessment process.

For government-funded support, yes, an assessment establishes what you need and what you are approved for. If you would rather not wait, or you are not eligible, private services can usually start sooner. Talk to us about which makes sense for your situation.

It depends on the type of support, your location, and whether your funding is already in place. We will give you a realistic timeframe rather than an optimistic one, and tell you promptly if we cannot meet it.

Absolutely, as much or as little as you want. We can include family in planning and reviews, or keep things strictly between you and your care team. It is your decision, and you can change it.
