If you're arranging or recommending in-home support for an older person, this page sets out how we work with referrers and what happens once a referral reaches us.

How we work with referrers
We aim to be straightforward to refer to: a clear intake point, a real person to speak with, and an honest answer about whether we can help.
If we can't meet the person's needs, because of location, capacity, or the type of support required. We'll tell you promptly rather than leaving a referral sitting.
What happens after a referral
- We acknowledge the referralYou'll know it's been received rather than wondering whether it landed.
- We contact the personWe speak with the person (and their family or representative where appropriate) about what they need and want.
- We confirm what we can offerIncluding whether we can cover their area and the supports they're after. If we can't, we say so.
- Support is planned and startedServices are arranged around assessed needs, preferences and goals. See Care Management.
Consent and information sharing
Please confirm the person is aware of and agrees to the referral before sending it, unless you're acting under a formal authority to do so.
Send only what we need to make contact and understand the general support required. Please don't send clinical records, discharge summaries or detailed health information through the web form. We'll arrange a secure channel if clinical handover is needed.
